*If you are trying to add a child to your Parent Profile for RIS - have the parent who completed RIS log in, click Profile, scroll down to Child Management, and then add the new player with their full name and birthday.

If there are multiple players, you may need to use their Hockey Canada #, which you can find on Teamsnap.

Tournaments

Team Tournament Permit HEO*

*This Form must be completed by every team. It must list ALL of the tournaments your team is registered in for the season with complete and accurate websites/contact info for the tournament. If your team is from the U9 Division, the form must articulate which tournaments are Half-Ice/Full-Ice.

**Once the form has been completed, have your Convenor review for accuracy. Once it has been approved by the Convenor, please email the form to president@leitrimhockey.ca for signature and forwarding to the District Chair.

***Rosters need NOT be submitted with Team Tournament Permits.
